Medicine 2023: New Therapies
Munich Clinic

Conservative, minimally invasive and surgical treatments at the Munich East Spine Center

This year, doctors will be able to help their patients even better than before. Neurosurgeon Dr. Samer reports on medical innovations, state-of-the-art techniques, and treatment methods for back pain patients.

Spinal canal: New multi-catheter eliminates constrictions without surgery

In cases of the dreaded spinal stenosis, doctors often recommend surgery. But now it's also possible to treat the condition without an operation. A new video catheter, which performs several functions simultaneously, can avoid the dreaded procedure in many cases. Through a small natural opening at the back of the sacrum, the doctor inserts the ultra-thin catheter into the epidural space. This is the area that surrounds the spinal cord.

Unlike conventional spinal catheters, which were only used to inject medication, the new multi-catheter can do much more. Using a control unit, the physician can move the flexible catheter tip 360 degrees and guide it with millimeter precision to any desired point under direct visualization.

Using a tiny lens, the high-tech device transmits high-resolution images directly from inside the spinal column to a monitor, magnified many times over. For the first time, the doctor can navigate with pinpoint accuracy and see exactly where changes are present. Additionally, fine instruments can be inserted through two working channels.

A small inflatable balloon can be used to loosen narrow passages and loosen adhesions.

A radio frequency electrode loosens adhesions and scar tissue, removes scar tissue, and soothes inflamed nerve roots and pain-conducting fibers.

An electric micro-knife cuts through tissue and removes larger adhesions.

A built-in rinsing and suction mechanism cleans the epidural space and suctions away removed tissue.

Medications injected through the catheter loosen adhesions, shrink intervertebral disc tissue, and eliminate inflammation.

Minimally invasive spinal therapies in Munich at the WZMO

Neurosurgeon and back specialist Dr. Samer Ismail from the Munich East Spine Center, who already uses the method, says: "This allows virtually all the steps of the procedure to be performed from the inside, just like in open surgery, without having to open the back. This is possible under local anesthesia and twilight sedation for spinal stenosis, herniated discs, chronic back pain, inflamed nerve roots, and pain following previous surgeries, both on an inpatient and outpatient basis.".
For example, pain in inflamed nerve roots can be gently soothed, medication can be delivered precisely to the source of the pain, and excess tissue can be suctioned away. Even the "shrinking" of herniated discs is possible. Unlike surgery, the minimally invasive treatment of herniated discs at the clinic in Munich requires neither general anesthesia nor an overnight stay.
